Just below your kneecap, over the soft tendon, about one finger-width down. Center the pad on the tendon, not on the kneecap or the shin bone.

Snug, not tight. Two fingers should slide under the strap. If your leg feels numb, tingly, or swollen, or your calf hurts, loosen it or take it off.
In deep knee bends any strap can shift. Check it between sets and re-center it. It's made for running, rope, and court work.
Yes. Wear it on bare skin or over a thin layer. If your skin is sensitive, a thin layer underneath helps.
Fold the loose end back flat onto the hook-and-loop so no hook side touches your skin. On smaller legs the end runs longer, so fold it all the way back.
For your training session. Take it off afterward, and don't sleep in it.
